Achilles Tendon Extension

Achilles tendon extension is a commonly used and effective soft tissue surgery for correcting plantar flexion foot deformity. Notes on Achilles Tendon Extension: 1. Achilles tendon extension can be used as an adjunct to the three-joint fusion (or ankle fusion), but it should be performed before the bone-joint fusion surgery and should not be performed at the same time, otherwise it will destroy The lever pressurization required by fusion surgery, instead, the plantar flexion deformity cannot be completely corrected. If reluctantly corrected, it will cause the sutured Achilles tendon to rupture, fail to heal or become too loose and cause the operation to fail. For the same reason, Achilles tendon elongation should not be performed at the same time as patellar tendon amputation. 2. When the inner and outer halves of the Achilles tendon are cut i If the plantar flexion is varus deformity, the medial half of the distal Achilles tendon should be cut off. This can reduce the strength of the medial Achilles tendon and correct the calcaneal varus. Conversely, the plantar flexion varus foot should cut off the distal lateral half. 3. When doing Achilles tendon extension, pay attention to the sural nerve accompanied by the small saphenous vein on the outside of the calcaneus and the posterior tibial nerve and posterior tibial blood vessels of the anterior medial side of the Achilles tendon to avoid injury. The affected foot has varus at the same time, and the lower Achilles tendon should be left on the outside of the calcaneus (or vice versa if there is valgus). 5. If the achilles tendon is cut below 90 °, try to cut the diaphragm and cut the posterior ankle capsule.
